     It would also be helpful if you would fill out the "Getting to Know You Form."  This will allow us to personalize your first dental visit, and to understand your specific desires and expectations.

    Due to HIPAA regulations and privacy concerns, these forms may not be completed on-line.  Instead, please print them on regular paper, fill them out, and bring them to your first visit with us.

    This will save you time when you arrive, and also give you a chance to consider your answers carefully, so that they are as complete and accurate as possible.
